Selected sessions will be recorded and available on audiotapes, CDs, and MP3s, which can be purchased one half-hour after the completion of these sessions. Orders may be placed and purchases made at the Audiocassette Tape Sales and Orders Booth in the Crystal Palace of the Jacob K. Javits Convention Center.
All cell phones and pagers must be turned off during sessions.
From 1:00 pm Monday, September 20, until noon Wednesday, September 22, registration counters will be reconfigured to provide a certificate of attendance for attendees. These certificates do not include continuing medical education (CME) credit information. To receive a certificate of attendance, complete and turn in the scientific evaluation form in your registration packet. Certificates will not be mailed after the meeting. CME hours will appear on the annual transcript automatically sent in January 2005 to all participants in AAO-HNSF-sponsored CME activities.
The Cyber Cafe will be located in the registration area of Hall 3E of the Jacob K. Javits Convention Center. Attendees will be able to access the Internet, check and send messages, and download the latest version of the official Annual Meeting Program to their PDA or Pocket PC.
The Resource Center and Store, booth 537, in the OTO EXPO will display all educational products including the Home Study Course, Self-Instructional Packages (SIPacs), Slide Lectures, Maintenance Manual for Lifelong Learning, monographs, and patient leaflets.

Tables will be available in the registration area, Hall 3E of the Jacob K. Javits Convention Center to display brochures for otolaryngology-related meetings. Nonmeeting brochures are not permitted on these tables.
This year we will begin a new program called “Mini-Courses.” In this program, an instructor will give a course in a small conference style room with attendance limited to 15 people. This program will allow a more informal arena for exchange of information. Instruction course fees will apply.
Scientific posters will be in Hall 3B of the Jacob K. Javits Convention Center and can be viewed throughout the meeting. Research Forum posters will also be in Hall 3B and can be viewed on Monday and Tuesday. Scientific and Research Posters are supported by Gyrus ENT.
The reception will be held at the Marriott Marquis in the Broadway Ballroom starting at 6:00 pm. You will have an opportunity to meet and greet old friends as well as make new professional acquaintances. Badges are mandatory. If your spouse/guest has not registered, please do so before going to the reception, entering the OTO EXPO, or attending scientific sessions. The Section for Residents and Fellows will hold its annual General Assembly on Monday, September 20,4:00-7:00 pm, at the Jacob K. Javits Convention Center, New York. Assembly highlights include an open discussion forum led by rep-resentatives from the American Board of Otolaryngology to discuss board certification and other issues of interest to residents. Following the open forum, the Section will elect its new leaders, discuss resolutions, and distribute awards.
No recording devices of any kind are permitted in meeting rooms. This includes audio, video, and digital recording equipment.
All attendees will receive an ExpoCard with their registration materials. The card will facilitate the exchange of information between the attendee and the exhibitor. The exhibitor will insert the ExpoCard into a reader, which will retrieve your name, address, telephone and fax numbers, and information about your practice or specialty. It will print an entry form for the Foundation prize drawings in the AAOHNSF Resource Center, booth 537.
The ExpoCard entry forms are your chance to win a trip for two to the 2005 Annual Meeting & OTO EXPO in Los Angeles, California. Multilingual personnel will be on hand with helpful information and a friendly smile. Academy members will volunteer as greeters to make our international visitors especially welcome.
Tickets for instruction courses will be sold in the registration area in Hall 3E of the Jacob K. Javits Convention Center. In advance, instruction course fees are $35 per hour and $50 per hour for hands-on computer courses. Onsite instruction course fees increase to $50 per hour and $75 per hour for hands-on computer courses.
You are invited to the OTO EXPO Reception on Monday, September 20, from 5:00 pm to 6:30 pm to visit more than 300 exhibitors in the exhibit hall.
Continuous shuttle bus service will be provided only from the four official hotels to the Convention Center throughout the meeting.
Smoking is prohibited in meeting rooms, exhibit halls, and the registration area.
Speakers can use only electronic slides and files to present at the Annual Meeting & OTO EXPO.
All papers read before the Foundation Annual Meeting become the property of the Foundation. All papers presented in the Plenary (Scientific) Sessions, and all AAO-HNSF-sponsored research, are required to be submitted to Otolaryngology-Head and Neck Surgery.
These papers are subject to editorial (peer) review and will be published at the discretion of the editor. If completed manuscripts are not submitted before the annual meeting, papers must be submitted onsite before presentation. The editor's office will be in the registration area, Hall 3E, of the Jacob K. Javits Convention Center.
