Abstract

OBJECTIVE
To assess the effect of systemic & local medication with ciprofloxacin on rhinoscleroma using electron microscopy.
METHOD
Study design: Prospective study. 18 patients having rhinoscleroma in its granulomatus stage were included in this study. Diagnosis was done depending upon endoscopic, bacteriological & histopathological data. Patients were divided into 2 groups: 1) Twelve patients treated with oral ciprofloxacin tablet 750 mg for 6 weeks. 2) Six patients treated with local ciprofloxacin in the form of 2% ointment applied to nasal granuloma daily for 6 weeks. After six weeks, biopsies were taken from the nasal lesions without local anesthesia or adrenalization. Specimens were prepared for EM study.
RESULTS
EM examination of nasal mucosa: Epithelium: Showed metaplasia with hydropic degeneration. Mikulicz cells: markedly decreased in size and number, mitochondria were swollen and vacuoles appeared empty of scleroma bacilli. Few bacilli were seen surrounded by fine granular material, others were seen empty without any cytoplasm. Plasma cells: increased in number and showed hyperactivity. Mott cells and Russell's bodies were increased Fibroblasts: showed marked activity.
CONCLUSION
Both local & systemic use of ciprofloxathin have been proved to be equally effective in treatment of rhinoscleroma.
