We report an inadvertent inaccuracy in our reply to J Lennane in the June issue of this
journal.[1] The
second last paragraph should read: ‘Dr Lennane claims considerable credit for
instigating and influencing [not ‘advising’] the NSW Mental Health Sentinel Events
Review Committee’
