Abstract
Purpose
This integrative review was conducted to provide a comprehensive picture of the use of standardized nursing languages (SNLs) in long-term care (LTC) systems.
Methods
A comprehensive search was performed with terminological variants of “standardized terminology” and “long-term care” in eight electronic databases up to December 2021. Eligible studies were further identified by screening the reference lists of publications that met the inclusion criteria. The quality of the included studies was appraised using the Joanna Briggs Institute checklists. The study findings were organized into themes, which represent the focus of the study.
Findings
Eighty-one publications that studied 12 SNLs in 17 countries are presented in this review. The Omaha System, NANDA-I, NIC, and NOC were the most common SNLs. Study foci were classified into five themes: evaluating the applicability of 10 SNLs (n = 22), characterizing nursing care using six SNLs (n = 16), developing core sets and tools based on seven SNLs (n = 15), documenting nursing care by using four SNLs (n = 14), and implementing intervention programs based on six SNLs (n = 14).
Conclusions and implications for nursing
SNLs can be used for various purposes, and the available evidence supports the expansion of their utilization. Further studies should continue to identify gaps in the existing versions of SNLs to reflect the LTC nursing process in multiple societies. Additionally, the successful use of SNLs requires background knowledge of nursing informatics; therefore, preparation should be started in the nursing curriculum and continued in healthcare facilities, including LTC settings. These research findings will assist healthcare managers, researchers, and policymakers in the LTC field in effectively utilizing SNLs.
목적:
본 연구는 장기요양에서 사용되는 표준 간호용어 유형에 대해 파악하고 효과적인 도입에 대한 기초자료를 제시하기 위하여 수행된 통합적 고찰이다.
방법:
8개의 전자 데이터베이스(PubMed, Embase, Web of Science, CINAHL, Cochrane Library, ProQuest, Scopus, Google Scholar)에서 “standardized nursing language”와 “long-term care”의 용어를 조합하여 2021년 12월 9일까지의 문헌 검색을 수행하였다. 다음 단계로는 검색된 문헌의 참고 문헌 목록을 활용하여 본 연구의 포함 기준에 부합하는 문헌을 추가하였다. 포함된 연구의 방법론적 질 평가는 JBI 질 평가 도구를 사용하여 비평적 분석을 수행하였다. 최종적으로 포함된 문헌에서 추출된 자료는 본 연구의 목적에 부합하는 주제로 도출되었다.
결과:
본 연구는 1990년부터 2021년까지 출판된 81개의 문헌이 포함되었다. 17개국에서 사용된 12개의 표준간호용어를 확인하였고 대부분 연구에서 일반적으로 활용하는 표준간호용어는 Omaha System과 NANDA-I, NIC, NOC이었다. 연구결과는 5개의 주제로 분류되었다: (1) 22개의 연구는 장기요양 영역에서 10개의 표준간호용어에 대한 대응성, 민감도, 타당도 및 신뢰도를 평가하여 활용가능성을 검증하였다. (2) 16개의 연구는 간호 진단(75%), 간호 중재(50%) 및 간호 결과(19%)를 포함하여 간호를 나타내기 위한 6개의 표준 간호용어를 활용하였다. (3) 15개 연구에서는 장기요양 영역에서 주로 사용되는 7개의 표준 간호용어를 기반으로 표준화된 용어셋트(n = 10), 사정도구(n = 3), 네비게이션(n = 1) 및 간호계획 양식(n = 1)과 관련된 도구를 개발하였다. (4) Omaha System과 NANDA-I, NIC, NOC을 사용한 14개의 연구를 제외한 나머지 연구는 전자기록을 사용하여 간호기록을 작성하였다. 2개의 연구는 전자기록방법과 수기기록방법의 표준 간호용어 활용 효과를 비교하였으며, 두 연구 결과 모두 전자기록방식이 더 효과적으로 활용되고 있음을 보고하였다. (5) 14개의 중재연구는 표준 간호용어체계를 기반으로 하는 개별 맞춤형 간호 계획이었다. 중재 연구에서는 대상자의 건강 및 건강 관리 행위를 개선시키는 것으로 보고되었지만 대부분의 연구는 비교군이 없어 제한점이 있었다.
결론:
본 연구는 장기요양 영역에서 표준 간호용어를 활용하기 위한 기초자료를 제시하기 위하여 시도되었으며 표준간호용어의 사용 범위를 파악하였다. 본 연구 결과 장기요양 영역에서 표준간호용어로 다 표현하지 못하는 영역도 있었다. 추후 연구에서는 다양한 환경에서 장기요양의 간호 과정을 반영할 때 표준 간호용어의 유연성과와 확장성을 검토하여야 한다. 표준간호용어는 간호의 특성화, 장기요양에 사용되는 핵심 세트/도구 개발, 간호기록 및 간호 지식 개발을 포함한 다양한 영역에서 사용할 수 있다. 본 연구결과는 표준 간호용어의 사용을 촉진하는데 기여할 것이다. 표준간호용어를 효과적으로 사용하기 위해서는 간호 정보를 활용 할 필요가 있으며 장기요양 간호실무에 대한 보다 적극적인 개입이 필요하다.
