A patient presented with an amelanotic triangular lesion in the right iris and ciliary body at the 9 o’clock position (Figure A). Gonioscopy and ultrasonographic studies were performed. Complete full-thickness resection of the lesion was achieved, leaving an iridian defect (Figure B). Histopathology confirmed iris melanoma, and systemic staging was negative. Phacoemulsification with intraocular lens and artificial iris implantation both in the capsular bag, was subsequently performed. One month later, anterior dislocation of the artificial iris occurred (Figure C), requiring reoperation with suturing of one segment to the native iris (Figure D) to ensure the stability of the prosthesis.
