Abstract

Answer E. Test a urine dipstick
Pre-eclampsia describes new-onset hypertension with proteinuria in a lady who is more than 20 weeks pregnant. Therefore, a urine dipstick should be done in clinic to check for protein. A 24-hour sample can be sent off but evidence of protein on a urine dipstick would suggest a referral to the obstetric team on the same day (not the medical assessment unit). Clotting screens are not normally affected in this condition. Labetalol is the drug of choice rather than ramipril for women with high blood pressure in pregnancy, but testing a urine dipstick is the most appropriate next step.
