Answer D. Urgent referral for surgical excision
The clinical picture is suggestive of a phyllodes tumour. They are rare stromal tumours that account for 1% of breast tumours. They most commonly follow a benign course with an unfortunate few that are malignant and can metastasise. These tumours can occur in any breast quadrant and up to 20% of patients can have axillary lymphadenopathy. Patients should be referred urgently to secondary care as management requires excision.
