This patient presents multiple red flags: History of prostate cancer, persistent back pain and subtle systemic features (fatigue, anorexia). While he has not volunteered red-flag symptoms explicitly, his multiple contacts, cancer history and reluctance to attend raise concern for serious underlying disease, including metastatic spinal involvement. The ‘three strikes’ rule reinforces the need for in-person review at the third adult contact. Clear, empathetic explanation is often required to overcome reluctance.
