Answer A. Carpal tunnel syndrome
Carpal tunnel syndrome is a common condition in wheelchair users, particularly elite athletes, owing to repetitive strain and pressure on the wrists. It typically presents with nocturnal numbness and tingling in the median nerve distribution (thumb, index, middle fingers), as described in this case. Ulnar nerve entrapment affects the ring and little fingers, while cervical radiculopathy is less likely in the absence of neck symptoms. Complex regional pain syndrome would be associated with pain out of proportion and autonomic features.
