Answer A. Dapagliflozin
Sodium-glucose co-transporter 2 (SGLT2) inhibitors, such as dapagliflozin, are recommended for patients with type 2 diabetes and chronic kidney disease because they reduce the risk of chronic kidney disease progression, heart failure and cardiovascular events independent of glucose lowering. Evidence from large trials shows benefits in patients with reduced glomerular filtration rate (eGFR) and albuminuria. In primary care, SGLT2 inhibitors should be considered in patients with chronic kidney disease and albuminuria who have an adequate eGFR, typically ⩾25–30 mL/min/1.73 m2 depending on guidance and are already receiving angiotensin converting enzyme inhibitor or angiotensin II receptor blocker therapy.
