In patients with suspected metabolic dysfunction-associated steatotic liver disease (MASLD), non-invasive fibrosis assessment is recommended to stratify risk. The Fibrosis-4 (FIB-4) score is commonly used as an initial screening tool in primary care. Patients with intermediate-risk Fibrosis-4 (FIB-4) results should undergo transient elastography (FibroScan®) or other elastography-based imaging to assess liver stiffness and detect significant fibrosis. Liver biopsy is generally reserved for uncertain cases or when non-invasive tests are inconclusive. This stepwise approach helps identify patients who require specialist hepatology referral while avoiding unnecessary invasive testing.
