A 64-year-old man presents with fatigue and reduced exercise tolerance over the past 6 months. He has a past medical history of hypertension treated with amlodipine 5 mg once daily and takes ibuprofen 400 mg intermittently for knee pain.
Recent blood tests show:
Haemoglobin: 94 g/L
Mean corpuscular volume (MCV): 72 fL
Ferritin: 8 µg/L
A. Begin oral iron therapy only and repeat blood tests in 3 months
B. Check serum vitamin B12 and folate levels
C. Refer urgently for upper and lower gastrointestinal investigation
D. Request haemoglobin electrophoresis
E. Stop ibuprofen and repeat ferritin in 6 weeks
