Trend arrow enhanced meal-time insulin calculation:
Total premeal insulin dose = food ± correction ± trend arrow adjustment.
The meal dose of insulin incudes the carbohydrate dose, the blood glucose correction dose, and the trend arrow dose.
Trend arrows can help determine optimal timing for premeal insulin.
If upward arrow at premeal glucose, consider instructing to take insulin 15 to 30 minutes before meal.
If downward arrow at premeal glucose, consider instructing to take insulin right before eating meal.
If premeal blood glucose is < 150 mg/dl and there is a downward trend arrow, instruct to consider holding the mealtime insulin until glucose has stabilized.
To avoid stacking of insulin, trend arrows should not be used during the postprandial time window. This can be further individualized to each person.
If the individual is having to make frequent adjustments based on trend arrow information, he or she may require a change to their treatment plan and may benefit from further diabetes education.
