Laminotomy is an established procedure to relieve symptoms of lumbar spinal stenosis. However, there is a group of patients with symptomatic recurrence. Re-decompression and fusion could be an effective salvage procedure but the results are seldom found in the literature. In this study, we focused on investigating the clinical outcomes and complication rates of revision decompression with fusion in this patient group.

A retrospective study including patients who had undergone revision decompression with fusion for recurrent symptoms due to same level restenosis after primary laminotomy for lumbar spinal stenosis was performed. Patients with recurrent symptoms due to prolapsed intervertebral disc, trauma, infection, and neoplasm were excluded. Demographics, clinical outcomes, and complications were retrieved.

Twenty-eight patients with a total number of 42 levels of revision decompression and fusion were included. With a mean follow-up time of 27 months after revision surgery, there were statistically significant improvement of 63, 49, and 13% in Japanese Orthopaedic Association score, visual analog scale for leg pain, and Roland-Morris disability questionnaire score, respectively. There were 6(21%), 2(7%), 0(0%), and 2(7%) cases of dural tear, infection requiring reoperation, new neurological deficit, and other complications, respectively, in these revision cases.

Bearing potential complications in mind, re-decompression with fusion is a viable option with reasonable clinical outcomes for patients with recurrent symptoms after laminotomy for lumbar spinal stenosis. As a treatment option for symptomatic lumbar spinal stenosis, primary laminotomy could have the potential benefit of lower complication rates in revision surgery.

The combination of ipsilateral floating shoulder and flail chest is a unique injury pattern that has not been previously described in the literature. We termed the injury pattern floating flail chest (FFC). The purpose of this study was to evaluate the effect of operative treatment of the shoulder girdle component to overall hospital length of stay (LOS).

Forty-one patients were enrolled between two level I trauma centers identifying with a combination ipsilateral floating shoulder and flail chest injury, 23 treated with operative stabilization and 18 treated non-operatively. This retrospective cohort study evaluated the overall LOS and intensive care unit (ICU) days.

The operative group had decreased overall LOS (10.1 vs. 19.8 days, p = 0.02) and decreased ICU days (3.4 vs. 10.3, p = 0.04).

This study describes a unique injury pattern that combines the floating shoulder and flail chest, FFC. Our study suggests that operative treatment of the shoulder girdle may decrease both overall LOS and ICU days in patients with FFC.

This article describes the translation and cultural adaptation of the Multi-disciplinary Rehabilitation Outcome Checklist Scale in Italian and reports the procedures to test their validity and reliability.

The forward and backward translation was conducted by specialized and certified translators, independently from each other. The scale was then reviewed by a group of 20 experts. The process of cultural adaptation and validation took place on a cohort of patients who performed a joint replacement of hip and/or knee surgery in two hospitals of Rome.

The culturally adapted scale was administered to 114 patients. It results that the interoperator reliability is equal to intraclass correlation coefficient (ICC) = 0.977 for hip and ICC = 0.97 for knee. The construct validity and the responsiveness are statistically significant.

It is a scale capable of assessing the patient in a comprehensive and multidisciplinary manner at the time of hospital discharge, useful for dismissing the patient in the most appropriate timing and with the best clinical and functional conditions.

To adapt the Rotator Cuff Quality of Life (RC-QOL) questionnaire into traditional Chinese (Chi-RC-QOL) and to evaluate the validity and reliability in patients suffering from rotator cuff pathologies in Hong Kong.

The Chi-RC-QOL will be constructed using the forward-translation followed by the backward-translation method. Thirty consecutive patients with clinically and radiologically confirmed rotator cuff pathology were recruited. Descriptive statistics will be followed by validity assessment using comparison with the Constant Shoulder (CS) score, University of California Los Angeles (UCLA) Shoulder Rating Scale, and Western Ontario Rotator Cuff (WORC) score. Parametric data will be tested using Pearson’s correlation coefficient for the total scores. Reliability was assessed using a test–retest interval of 30 min. The Cronbach’s α and intraclass correlation coefficient were calculated.

Cronbach’s α and internal consistency scores were high for all parts of the Chi-RC-QOL, with Cronbach’s α ranging between 0.89 and 0.98. Internal consistency scores range from 0.82 to 0.92, which can be regarded as an excellent correlation. Test–retest reliability was excellent for all parts of the Chi-RC-QOL with good absolute reliability. Chi-RC-QOL correlates well with the CS score, UCLA Shoulder Rating Scale, and the WORC score, with all being statistically significant.

The current study adapted the RC-QOL to traditional Chinese version. The analysis confirmed the validity and reliability of the Chi-RC-QOL questionnaire.

Lack of local data concerning the long-term clinical outcomes of scarf osteotomy in Hong Kong.

This is a retrospective review of 75 patients (88 feet) who underwent scarf osteotomy with a mean follow-up of 94.20 months (60–144 months). Clinical ratings were based on visual analog scale (VAS) and American Orthopaedic Foot & Ankle Society (AOFAS) scale. Weight-bearing radiographs were used to perform angular measurements: (i) hallux valgus angle (HVA), (ii) intermetatarsal angle, (iii) distal metatarsal articular angle, and (iv) tibial sesamoid position before operation and at the time of latest follow-up.

The mean overall AOFAS scales improved from 29.66 (5–49) points preoperatively to 86.83 (39–100) points at the time of final follow-up. The VAS improved from 6.61 (5 to 9) to 0.66 (0 to 4). The average HVA correction was 25.42 (8–46). The 8- to10-year recurrence rate (HVA > 20) was 31.80%. A higher degree of preoperative HVA predicts higher recurrence rate.

Scarf osteotomy is an effective procedure for symptom control and improvement in radiological parameters for hallux valgus deformity. However, recurrence rate is relatively high.

In this study, we assessed functional outcomes among olecranon fixation techniques used following transolecranon approach in patients with comminuted fracture of distal humerus.

The study included 37 elbows that underwent olecranon osteotomy due to distal humerus fracture. Functional outcomes were assessed among patients who underwent fixation via tension band technique, plate, or screw fixation.

The implant irritation was seen in 18% of patients who underwent K-wire plus tension band fixation, in 75% of patients who underwent olecranon plate plus screw fixation, and in 18% of patients who underwent cancellous screw fixation. The rate for implant removal was 2.6 folds lower in K-wire plus tension band fixation when compared to olecranon plate plus screw fixation.

We concluded that K-wire plus tension band technique and cancellous screw fixation performed in accordance with technical principles caused less skin problem when compared to other techniques. In olecranon fixation, both methods with enhanced stability by tension band can be preferred.

Sarcopenia and osteoporosis increase the risk of fall, resulting in fragility fracture. Intervention programme on sarcopenic hip fracture was set up in our centre.

All patients aged ≥65 admitted to Caritas Medical Centre with operatively treated hip fracture and diagnosed with sarcopenia were included. Interventions include inpatient nursing education, dietary advice, therapist assessment and structured 12-week Geriatric Day Hospital (GDH) exercise programme. Those who are not eligible for GDH training were classified as control group. Changes in relative skeletal muscle mass index (RASM), muscle strength and functional scores were measured.

There were 11 intervention and 13 control patients. Both groups of patients were noted to have improvement in RASM, knee extension power of lower limbs and functional scores. Between-group analysis showed there was more improvement in injured limb extension power in the intervention group, although it is statistically insignificant (control group: 3.585 kg, intervention group: 5.827 kg, p = 0.147). There was no statistically significant difference in the 3-month change in RASM (control group: 0.581 kg/m², intervention group: 0.347 kg/m², p = 0.369) and functional scores.

These sarcopenic hip fracture patients may be too frail to benefit from traditional exercise treatment for sarcopenia. However, more improvement in muscle strength was observed in the intervention group although not statistically significant. Intervention programme with exercise prescription in sarcopenic hip fracture patients may have the potential to improve the lower limb muscle strength. A more intensive and longer exercise training programme with incorporation into day rehabilitation model may be needed for these frail hip fracture patients.

Acute rupture of the distal biceps tendon is relatively uncommon, and surgical repair is advocated for acute injuries to restore strength and function. Numerous techniques are described in the literature, with no true consensus regarding the optimal method of fixation. The aim of this study was to evaluate patient-reported outcome measures, clinical outcomes and complications for patients undergoing distal biceps tendon repair using two fixation techniques: cortical button and suture anchor.

A retrospective single-unit case series of 51 cases (50 patients) underwent distal biceps tendon repair, comprising 19 cortical button and 32 suture anchor fixations. Patients were assessed using the Disabilities of the Arm, Shoulder and Hand (DASH) questionnaire and overall satisfaction. Clinical outcomes and complications were reviewed from patient records.

Thirty-eight (76.5%) patients responded to DASH questionnaires with a mean follow-up of 189 weeks (11.5–425 weeks). Mean DASH scores for cortical button and suture anchor groups were 6.2 (0–30.8) and 3.3 (0–16.7), respectively (p = 0.21). Eight patients (16%) reported lateral cutaneous neuropraxia in the early post-operative period. All but two of these patients experienced complete resolution at the final follow-up. One patient had heterotrophic ossification, which did not require any further intervention. There was one re-rupture (suture anchor), but the patient declined further surgical intervention.

There were no statistically significant differences in the patient-reported outcome measures, overall satisfaction and complication rate between patients undergoing either method of fixation.

The direct anterior approach (DAA) for hip arthroplasty has gained popularity in recent years with numerous articles suggesting that short-term outcomes are advantageous over alternative approaches. The ‘bikini’ inguinal crease line incision is a novel variant of the DAA, which reduces the scar.

We conducted a retrospective cohort study to assess functional and general health outcomes and complications of the two different surgical incisions used in DAA total hip arthroplasty (i.e. ‘bikini’ and longitudinal incisions).

We found no statistical difference between the novel bikini incision and the traditional DAA hip arthroplasty.

This approach offers a cosmetically improved scar with no compromise in outcomes.

A retrospective study to compare the rate of facet joint violation (FJV) in lumbar posterior spinal instrumentation using open pedicle screw, percutaneous pedicle screw, and cortical bone trajectory (CBT) technique. CBT is a new posterior spinal instrumentation technique in which a more caudal entry point can minimize iatrogenic damage to the cranial facet joint. Only one recent study reports incidence of FJV of 11%; however, no previous reports comment on radiological outcomes comparing to traditional open and percutaneous screws.

We reviewed 90 patients who underwent lumbar posterior spinal instrumentation from January 2016 to June 2017. Postoperative computer tomography scans were performed to evaluate FJV. Incidence of FJV was graded by three reviewers according to Seo classification.

Totally, 446 screws (open 43.4%, percutaneous 37.8%, CBT 18.9%) were inserted. Among these, 6.3% (28/446) had screw head or rod in contact with facet joint and 0.9% (4/446) had screws directly invaded the facet joint. Overall, FJV was 7.2% (CTB = 3.4%, open = 10.4%, and percutaneous = 4.5%, p = 0.075).

CBT technique has potential advantage in reducing FJV. It has a unique entry site at lateral aspect of pars interarticular with a caudomedial to craniolateral pathway. It is a reasonable alternative to open or percutaneous techniques in lumbar posterior spinal instrumentation.

In the field of cartilage repair, cell-based therapy, employing the use of mesenchymal stem cells and chondrocytes maybe a propitious treatment option. Once transferred, success of transplanted cells is determined by their immunogenicity making assessment imperative. Another contender in the field, may be articular chondroprogenitors (CPs) shown to possess chondrogenic potential and reduced expression of hypertrophy markers. Our aim was to assess immunogenic properties of CPs (non-diseased and osteoarthritic (OA)) and compare them with chondrocytes since it may be a good alternative for cell-based therapy and data regarding its immunogenic profile is limited.

Human chondrocytes and CPs from the same cartilage source were isolated. Passage 0 cells characterized by fluorescence-activated cell sorting against human surface antigen were human leucocyte antigen class I (HLA-A2 and HLA-B7), HLA-DR and its costimulatory molecules (CD80 and CD86) and macrophage/monocyte marker (CD14).

Our observations indicated that CPs isolated from human non-diseased and OA cartilage showed similar immunogenic properties to chondrocytes isolated from the same source with no significant difference in expression. High-to-moderate expression of MHC class I (HLA-A2 and HLA-B7) and moderate-to-low expression of MHC class II (HLA-DR and its co-stimulatory molecules CD80 and CD86) were observed. This is the first report to shed insight on the immunogenic properties of human cartilage-derived progenitors, a potential contender in the field of regenerative therapy for formation of genuine hyaline cartilage.

Intensive rehabilitation of individuals with bilateral lower limb loss poses a great challenge to both rehabilitation team and amputees themselves due to unavailability of a sound leg to provide stability in standing and gait. Although gait characteristics of individuals with unilateral transtibial amputations are well documented in the literature, very less is known about those with bilateral limb loss.

To examine the gait characteristics of an individual with bilateral transtibial amputation (BTA) and its comparison with an able-bodied (AB). This study also provides a real-life presentation of successful prosthetic rehabilitation.

Temporal–spatial, kinematic and kinetic gait parameters were analysed for a 45-year-old male individual with traumatic BTA using prosthesis in a motion analysis laboratory setting with force platform (BTS P-6000) and cameras with reflective markers (BTS SMART-DX6000).

Variances in many temporal–spatial, kinematic and kinetic parameters were observed. The findings of temporal–spatial parameters revealed that the individual with BTA walked with slower speed, lower cadence, shorter step lengths and wider step width compared to that of AB. Ankle dorsiflexion, stance knee flexion and swing hip hiking were reduced in an individual with BTA compared to AB. In kinetics, he demonstrated low peak ankle muscle power, increased muscle power amplitudes and phase duration at the hip and knee joints compared to AB individual. The combination of an intensive prosthetic rehabilitation led to completely independent and remarkable degree of functional ambulation.
