A mother brings her 15-year-old son to your evening surgery as an emergency. He started with a headache, lethargy and muscle aching yesterday after returning home from school. Today he has been vomiting and has developed a fever. His mother is now worried as he appears to have become confused this afternoon. On examination, he is alert but with intermittent confusion.
